Nominate someone you know for Citizen of the Year   The Citizen of the Year Award is given annually in recognition of outstanding and unselfish service to the community of San Luis Obispo.  Click here to download the nomination form. Anyone may be nominated for the award, but only Chamber members may nominate.  The information you provide will help the Chamber Board in their decision process.  Only completed forms will be accepted.  You may attach additional information. Nominations should be kept confidential. The award recipient will be kept closely guarded until it is announced at the Chamber’s Annual Dinner on January 23rd, 2010.   Please return all completed forms to the San Luis Obispo Chamber of Commerce, Attn: Lindsey Miller, 1039 Chorro Street, San Luis Obispo, CA 93401 or fax it to (805) 543-1255.  All forms are due by 5pm on Friday, November 20th.
