The Dignity Health Central Coast hospitals, including French Hospital Medical Center (FHMC), are now accepting applications for Dignity Health Community Grants.

The Dignity Health Community Grants Program partners Dignity Health Central Coast hospitals, including FHMC and Marian Regional Medical Center’s (MRMC) Santa Maria and Arroyo Grande campuses, with other nonprofit organizations working to improve the health and quality of life of the communities they serve and which exemplify Dignity Health’s five core values. These core values include: dignity, collaboration, justice, stewardship, and excellence.

In order to be considered for funding, projects must involve collaboration with other organizations, must benefit those served by Dignity Health Central Coast facilities, and must also address all five of the following principles: disproportionate unmet health related needs, emphasis primary prevention, contribute to a seamless continuum of care, build community capacity, and emphasize collaborate governance.

Grant funds are to be used to provide services to underserved individuals, and the proposed program must also integrate one or more of the following four strategic priorities identified in the Central Coast Service Area Community Health Needs Assessment report.

  • Access to Healthcare – overcoming barriers to access, including health insurance status.
  • Emergency Room Utilization – enhancing primary care utilization, such as wellness clinics and navigator programs.
  • Clinical Conditions – providing awareness, education and support to deal with obesity, diabetes, heart disease, stroke hypertension, oral health or mental health.
  • Mental Health – creating access to culturally appropriate, bilingual counseling services.

Those who believe their organization meets these expectations are invited to submit a Letter of Intent using the below link.  (http://www.dignityhealth.org/cm/content/pages/apply-for-a-grant.asp)
